Targeted Case Management

Targeted Case Management is a service that helps people who use specialized mental health, aging or disability services. Our case management team members work with you and/or your loved one to assess individual care needs, develop person-centered support plans and plans of care, and provide assistance in accessing and monitoring needed services.

Services include:

  • Facilitating community resource availability based on each individual’s needs to assure maximum community participation.
  • Monitoring support plans for residential, day services, and in-home support providers to make necessary improvements in supports provided.
  • Meetings as requested with the individual to whom supports are provided and with parent and/or guardian to ensure needs are being appropriately met.
